1. Objectives of funding
Mental disorders belong to the most important disease groups. They have a lifetime prevalence of 30 % and are often responsible for early retirement and sickleave. Due to the demographic development of the population in Germany it is expected that the frequency of mental disorders and their impact on the health care system will increase. Psychotherapeutic interventions of various kinds are routinely applied in the treatment of many mental disorders. To date, psychotherapeutic treatments play an important role in the national health care system. The aim of the present funding measure is to advance psychotherapy research, to improve the interdisciplinary cooperation in the field and to ameliorate the structural conditions. The accelerated transfer of scientific results into heath care is another goal. The funding measure supports nationwide networks, dedicated to a single specific disorder or a group of closely related disorders. Funding occurs within the framework of the Federal Government's "Health Research – Research for the People" programme and includes provisions for a network-wide organisation and communication structure as well as for specific research projects. The funded research networks are expected to perform high quality research, to have synergistic effects in terms of interdisciplinarity and cooperation between research and health care. In addition, they shall improve education of health professionals and provide benefits for the health care system.
2. State of funding measure
Since 2006, the Federal Ministry for Education and Research supports psychotherapy research in Germany by implementation of the funding measure „Psychotherapy Research”. Out of 31 submitted grant proposals, five nationwide networks have been chosen for funding by a two step evaluation procedure. In total, the ministry will allocate up to 25 mio Euros for a funding period of 6 years, divided in two 3yr phases.

Psychotherapy of Psychotic Syndrome

Psychotic disorders and in particular positive symptoms represent a major clinical challenge and economic burden. According to meta-analyses, cognitive behavioural treatment (CBT) has positive effects on the general mental state. However, the specific evidence for reducing positive symptoms has to be improved. This network is designed to conduct a full scale clinical trial addressing positive symptoms which is accompanied by related synergistic research projects. The network comprises process-outcome research, neurobiological research addressing basic questions of delusion formation and treatment research investigating extensions of CBT for special populations. Problems of transfer into routine care will be identified and addressed by projects on cost effectiveness.

The lifetime prevalence of eating disorders is approximately 5%; women are much more frequently affected. Eating disorders can be considered as developmental disorders because they usually become manifest during adolescence and often continue into adulthood. Accordingly, practitioners in child and adolescent psychiatry, adult psychiatry, clinical psychology and psychosomatic medicine typically treat these patients. The leading researchers of Eating Disorders in Germany collaborate in this network. The network involves three major treatment studies including patients with the “classical” eating disorders (anorexia nervosa and bulimia nervosa; AN, BN): 1. an outpatient treatment trial for AN, 2. a trial comparing inpatient and day patient treatment in adolescents with AN, 3. a relapse prevention trial in patients with AN and BN after discharge from inpatient treatment. Psychosocial variables will be assessed across the treatment trials. Furthermore the network addresses scientific questions concerning genetics, epigenetics, endocrinology and neurobiology. In addition, members of the network will continue to work on treatment guidelines for Eating Disorders.

Social Phobia Psychotherapy Research Network (SOPHO-Net)

A major aim of the proposed network is to improve the insufficient response rates of patients with social phobia (SP, for abbreviations see Glossary). For that purpose, refined manualized and disorder-specific forms of cognitive-behavioural therapy (CBT) and short-term psychodynamic psychotherapy (STPP) will be applied, thus focusing on the most frequently applied methods of psychotherapy in SP. The transfer of manual-guided psychotherapy from research settings into routine clinical practice will be explicitly addressed in specific projects. Studies of adults, children and elderly patients are included, and a specific focus is on women and female children with SP who suffer particularly from SP. Cost-effectiveness analyses will be applied to see if the treatments pay off. Psychological, genetic and neural parameters will be studied and their relation to treatment outcome will be assessed.

Panic disorder with agoraphobia (PD/AG) is the most costly and disabling of all anxiety disorders, characterized by a unfavourable natural course resulting in a diversity of secondary mental health problems. Although effective psychological and drug treatments are available, surprisingly few PD/AG patients are diagnosed. Although cognitive-behavioural psychotherapy (CBT) is the best studied and obviously most effective therapy for PD/AG, the essential active ingredients of CBT in the treatment of PD/AG and their relation to neurobiological fear circuit mechanisms are still poorly understood. This network represents a collaborative effort to elucidate these knowledge deficits by systematically employing an integrated approach that links targeted psychological interventions to the specific neurobiology of fear circuit mechanisms as made accessible by highly sophisticated technologies. A concerted stepwise action on different levels will be carried out, including epidemiological, clinical and basic research as well as the transfer into the public health system.

Attention Deficit Hyperactivity Disorder Network (ADHD-net)

Attention Deficit Hyperactivity Disorder (ADHD) is a serious mental disorder with a persistent pattern of severely impaired attention and concentration, hyperactive and impulsive behaviour, emotional instability, restlessness and disorganized behaviour. The prevalence in childhood is estimated to be about 5% and in adulthood the prevalence is indicated to 2 and 4%. Pharmacotherapy and to a limited extent psychotherapy are well established and evaluated therapeutic options in childhood ADHD, but are lacking for adult ADHD. Main focus of the network is the comparison of pharmacological treatment versus psychotherapy. The network will also examine whether there are specific neurobiological marker which can differentially predict therapeutic response to pharmacotherapy, psychotherapy or a combination of both. Besides Würzburg and Freiburg, five additional university centres are contributing to the network.
